Medical Records

It is our top priority to ensure your care continues. Your medical records are available to you and to the physician you designate. Since the records of your case are confidential, written authorization will be required to make them available to another physician.

All records for the practice are currently available through the Kettering Release of Information Department who can be reached at 937-762-1395.
